The procedure is as follows.
1. Please download 2 of the power of attorney form and sample PDFs. (You can choose letter size or A4 size.)
2. Please use the sample to fill out and sign both of the power of attorney. Then, please send 2 forms, one for the high court and the other for the highest court, to the address below by the end of May.
